To refer your patient, send a GP or optometrist referral letter to our specialist ophthalmologists.

Referring a Patient for Specialist Eye Care

We triage each referral by presenting condition, contact your patient directly to arrange the appointment, and report back to you after each consultation.

  • Referrals can be conveniently sent by fax , email or fill up form online and with a dedicated phone line also available for urgent cases as detailed below.

  • Our team includes experienced subspecialists covering cataract, glaucoma, medical and surgical retina, alongside general ophthalmology, as outlined in the routing guide.

  • To ensure we can triage your patient accurately and efficiently, please include comprehensive clinical details as set out in the section below.

  • For urgent presentations involving any of the listed red flags, please contact our clinic directly by phone rather than sending a standard referral letter.

You can send a referral by fax, email, fill up form online or by phone, and we accept standard GP and optometrist referral letters in every format.

When to Telephone Us for an Urgent Review

Urgent presentations warrant urgent specialist contact rather than a routine letter. For the red flags below, please telephone our rooms directly on (02) 9874 5947 so we can arrange same-day or next-day assessment.

  • Sudden loss of vision
  • Sudden onset of floaters or flashes with a curtain or shadow across the visual field
  • Suspected retinal detachment
  • Suspected acute angle-closure glaucoma (sudden severe eye pain, redness, haloes, nausea)
  • Sudden onset of distortion or a new central scotoma suggesting active wet AMD

Other presentations are semi-urgent rather than emergencies. These include new floaters or flashes without a field defect, raised intraocular pressure without acute symptoms, rapidly progressing visual field loss, or a rapidly enlarging eyelid lesion. Please note the urgency in your letter so we can prioritise triage accordingly.

What to Include in Your Referral Letter

A detailed referral letter lets us triage accurately and prepare for your patient's first consultation. Please include the following where available.

  • Patient name, date of birth and contact details
  • Presenting condition or concern, and urgency level
  • Relevant ocular and systemic history
  • Current medications, including topical eye drops
  • Examination findings: visual acuity, intraocular pressure, cup-to-disc ratio, OCT or visual field results, fundus findings
  • Preferred surgeon, if known

Nominate a surgeon by subspecialty if you have one in mind, or leave the triage to our staff. The fuller the picture, the better we can tailor the first appointment.

How We Report Back to You After Each Visit

You remain informed throughout your patient's episode of care. We send a clinic letter to the referring practitioner after each consultation, documenting our assessment, management plan and any recommended follow-up. Where clinical handover is needed, we will speak with you directly, and our staff can provide fee information at scheduling so you can advise your patient ahead of the visit.

Where We Consult and Operate

All consultations are held at our rooms at 22 East Parade, Eastwood NSW 2122. Epping Surgery Centre is our main day-surgery venue. Depending on the surgeon and procedure, operations may also take place at Sydney Eye Hospital, Macquarie University Hospital or Chatswood Private Hospital.
