Patient Registration

Completing your registration online before your appointment means our friendly staff already have your details when you arrive, so you spend less time at the front desk and more time with your surgeon.

Before You Start

This form takes about two minutes. It collects only the details our reception staff need to create your patient file and confirm your appointment.

You will need a current referral from your GP or optometrist to see a specialist ophthalmologist in Australia. You do not need to upload it here. Bring it with you, or ask your referring doctor to send it ahead by fax or email.

We do not ask for your medical history or your current medicines on this form. Your surgeon will go through those with you at your consultation.

If you would rather complete a paper form on the day, that is completely fine. Please arrive 15 minutes before your scheduled appointment time.

How We Handle Your Details

The details you enter here are sent to our reception team and stored so they can be matched to your appointment. We use them to prepare for your visit and to contact you about your care. We do not use them for marketing.
